Alpine Wellness Clinic — Mobile Messaging Service Agreement

By opting in to receive SMS messages from Alpine Wellness Clinic, you agree to these Terms and Conditions. Please read them carefully before subscribing. Message and data rates may apply. |
§ 01 Program Description
Alpine Wellness Clinic ("we," "us," or "our") operates an SMS messaging program that sends promotional offers, transactional updates, account alerts, and other communications to patients and customers who have opted in to receive such messages.
By providing your mobile phone number and consenting to receive SMS messages, you agree to be bound by these Terms and Conditions in addition to our Privacy Policy.
§ 02 How to Opt In
You may enroll in our SMS program through the following methods:

By opting in, you expressly consent to receive recurring automated marketing and informational text messages from Alpine Wellness Clinic at the mobile number you provide. Consent is not a condition of purchase or receipt of medical services.
§ 03 Message Frequency & Content

CONTENT TYPES | Appointment reminders, clinic events, sales, wellness opportunities & updates |
Message frequency may vary. You may receive fewer messages in quieter months and up to 5 messages in months with events, appointments, sales, or other opportunities. We reserve the right to modify message frequency at any time.
§ 04 Rates & Charges
Message and data rates may apply. Standard carrier rates for text messages and data usage set by your mobile carrier will apply. Alpine Wellness Clinic does not charge a separate fee for our SMS service, but your mobile carrier may charge you for receiving and sending messages.
Contact your wireless carrier for information about your plan’s messaging rates and data charges.
§ 05 How to Opt Out
You may opt out of the SMS program at any time. To stop receiving messages, reply with any of the following keywords:

After sending a stop command, you will receive a one-time confirmation message. No further messages will be sent to that number unless you re-enroll. You may also call 208-917-2772 to opt out.

§ 07 Supported Carriers
Our SMS program works with most major US carriers. Carriers are not liable for delayed or undelivered messages. Supported carriers include but are not limited to: AT&T, Verizon, T-Mobile, Sprint, Boost Mobile, Cricket, MetroPCS, U.S. Cellular, and others.
§ 08 Privacy
Alpine Wellness Clinic respects your privacy. Your mobile number and any related information will be used solely to provide the SMS service and will not be shared with third parties for their own marketing purposes without your consent. § 09 Limitation of Liability
To the fullest extent permitted by law, Alpine Wellness Clinic is not liable for any damages arising from your use of or inability to use the SMS service, including but not limited to delays, failed deliveries, or errors in message content. The service is provided “as is” without warranty of any kind.
§ 10 Changes to These Terms
Alpine Wellness Clinic reserves the right to modify these Terms and Conditions at any time. Changes will be effective upon posting to our website or notification via SMS. Continued participation in the SMS program following notification of changes constitutes acceptance of the revised terms.
